Patient Forms
In preparation for your appointment, our office will email you the New Patient Intake Forms in advance to fill out online and submit prior to your visit. You can also find the required forms here New Patient Intake Forms, where you can use your full name and phone number to access if you do not have an email address. All documents should be submitted prior to your appointment to minimize any delay on the day of your visit.

Handicapped parking and accessibility on North side of the building.
Via I-95:
- Exit on 79 East – PGA Blvd
- Merge onto PGA Blvd and continue to RCA Blvd (immediately following the Shell gas station – approx 300 yards)
- Make a right turn onto RCA Blvd (immediately following the Shell gas station – approx 300 yards)
- Continue for 1 1/2 blocks to the second building on the right with blue awning
Via Military Trail:
- Head East on PGA Blvd and continue under the I-95 overpass
- Make a right turn on RCA Blvd (immediately following the Shell gas station – approx 300 yards)
- Continue for 1 1/2 blocks to the second building on the right with blue awning
Via Alternate A1A:
- Head West on RCA Blvd to the Stop sign
- Make a right turn and continue for 1 block (this is still RCA Blvd)
- Our office is located on the left-hand side in a two story building with a blue awning

Out of Town Patients
Because of the beautiful location and unique nature of our practice, we often see patients from around the country as well as from around the world. Doctors from many countries have shadowed our doctor, and as a result, an international presence has been established. Cosmetic eyelid surgery, trauma, and eyelid complications following cosmetic surgery are the most common reasons for out-of-town patients to seek a consultation with our eyelid expert.
If you are coming to us from out-of-town, we are happy to assist you in your travel needs. We recommend you come to town the day before your surgery and plan on staying here through the early post-operative period, if possible. We recommend you travel with a companion who can assist in your recovery; however, if this is not possible, our office can find a nurse to assist you while you recover.
